It needs a shoulder strap, a sturdy shoulder strap with cushioning nothing fancy although a fancy one would be nice. I use the shoulder strap from an old bag an old fall leather bag that came from Temu works pretty well with that. I like the paisley looks pretty decent with it. I would definitely recommend this bag, but I would definitely recommend getting a handful of shoulder straps for these types of products and just keep them around the house for whenever you need them. They're interchangeable don't throw out shoulder straps. You always need them good bag, though gonna use it for my therapy tools just had a stroke and the other one since I bought two of them is for the medication‘s that I have to take so one for the therapy tools and one for the medication‘s both of them need shoulder straps, cause I had a stroke and it affected my left arm and my left hand. I have grip, but it's a little wobbly and I don't have dexterity and strength yet. I'm working on it. That's why I needed the bags.
