Same Problem - Hook Falls Off

We have four Eva tissue holders installed by the builder in our new home delivered 4 months ago. Already the hook is falling out of one and it's just a matter of time before the others fail. Hook seems to only be held in by a in cheap plastic collar. Very poor design and not up to Moen quality!

TP hook part falls off my Eva model Moen TP holder

We have four of these Eva TP holders , in brushed nickel, in our brand new home. Moen fixtures are in every bathroom and in the kitchen. The Eva TP holders hook has fallen off in three of the holders. Is there a way to fix, or are they all defective. Seems like I am not alone in the problem. Please help asap, this problem can be embarrassing when company comes.

Fell apart literally 2 pieces, 1 year old -

I have 3 of these about 1 year old and I had to replace one because the hook started squeaking then fell apart. I have a second one squeaking. I still have the broken one if you want it for quality check.
