Keep it cold by placing in the freezer but can be used without freezing as you don't add water to this. No water mess like you get if using a frozen water bottle. It lasts forever as I bought one a few years ago for plantar fasciitis and still have it. Adult or children can use it. I bought a second one to give to a kid having foot issues. It looks exactly like the picture.
