Faucet falls off

I have this in my bathroom, and the main faucet keeps falling off. I tighten the 2 tiny hex screws about once a week. The faucet itself is a heavy solidly built piece. It just does not stay in place.

Horrible

Because this product is discontinued I can not find a replacement for the shower head which out of the blue just popped off. To replace the entire system it's a small fortune. I emailed Kohler and no one got back to me.
