iphone 5 s

I ordered an iPhone 5 s, a phone case and purchased and extended warranty plan online. I received my items in the mail, no paperwork enclosed. No information on the 50.00 warranty I purchased. Not a happy customer. The phone works great right now but in the future if I have an issue I don't have any contact information regarding the warranty.
