I have got a 22 inch head and the medium was a might big too big. Since I love the hat I will keep it anyways. I was able to make it a bit snugger by re- tying the leather around the brim. I wouldn't bother with ordering the large unless you have an enormous head or are a very large man. I wish it came in a small size, oh well
