I have used computers all day every day since before there were mice. Over the years I have had to move to ergonomic keyboards and mice to relieve pain in my hands and wrists. The two that have worked best for me recently are this Adesso iMouse E10 and the Evoluent Vertical Mouse 4. The Evoluent has the hand almost completely vertical. That may not be the most natural for all users. The Evoluent has more features, three buttons and the wheel button, but costs more. This Adesso iMouse E10 has two buttons plus the wheel button. Both also include the two thumb buttons. The Adesso lets the hand rotate just a little more from vertical. For me that is just a little more natural. The Adesso is slightly lighter in weight. Both works great. I would recommend both. Unless you need the extra center button the Adesso iMouse E10 is a better bargain.
