This comes with four pads that can be attached to match the exact point of support that is individually required, yet even with the larger pad is not outwardly noticeable. I use the larger pad when I will be physically working and the smaller one when I am around the house and not exerting strenuously. I do not have other types to compare but the pocket type loses much of specific placement benefits. The straps located for me exactly where I need them to be, and can be tightened or loosened to hold the pads in place with one exception, if you loosen the strap beyond the edge of the hook retainer and the bottom of the brace, its edge will irritate somewhat and just a slight increase will prevent that. I have not had surgery so I can not comment on any of those issues. I place the brace with the pads attached where they need to be while lying on my back and holding them there with one hand, I rise and cinch the belt around my waist with the other hand. Placement to the front of the straps is adjustable for angle and contact support for the pads and also how much pressure to hold the end of the pad down with the exception noted above. I like the uniqueness and the adjust ability this brace offers. I some times forget that it is on, but it still feels good at the end of the day to take it off. I have not had problems when wearing the brace even during strenuous exertion, but forgetting to maintain support after taking it off and showering I have. I have purchased multiple of these to use when I will be sweating and dirty others when not or laundering. This works for me and I am glad that I bought them, especially since the surgery alternative is not for me.
