Size up! My dad is normally a medium, we got the large and it's perfect. The best fit. It doesn't absorb a crazy amount, but they do work. My dad is bed-ridden so it's harder to catch everything while he's laying down. May do a lot better if you're on the go and have leaks. Overall though we love! Best brand and only brand we use!
