I find the angle, density and firmness of the brush makes managing the back tooth more pleasurable. If you're using this for the first time, slow down your strokes; most brushes are either soft or medium; slowing down lets you get accustomed to it and should avoid or at least lessen tears on gums. After using this I find my medium bristles too soft.
