What I do
I use the medium size for my 42 year old handicapped son. He also uses an adult pull-up type diaper over it because he fidgets and pulls the tabs off the first diaper and most of the time, it catches any leakages . It's easier to change the tab diaper because you don't have to take his pants off to change him.
